I have some great news!  After reading the DOCS web site and discovering there is a FORMAL complaints process, I asked our support people if they had copies of the form.  They all questions what the form was,  never hearing of it before.

I decided it was time to send - yet another - fax to DOCS.  I simply asked for the form.

Hey Presto - here it is!  Ready for you to fill out!

Now this is what we are going to do.  You can either send us email and request us to FAX your a copy (don't forget your fax number) or ...

1. Click on the form on the right.
2. Print the page. 
3. Fill in Section 1 Person Making the Complaint
4. I'm not sure if we *should* fill in Section 2.  I'll leave that up to you.
5. Fill in Section 3 with the details about what your complaint is.
6. Fill in Section 4 with what you would like to see happen. (Be creative!)
7. Fill in the "Customer Name" on the bottom in the receipt section.
8. Either save it as a JPG or GIF and email it to me or fax me the printed copy.
9. I will take the forms in each day for the next few weeks and lodge them.
10. You will notice there is a receipt section on the bottom :)  I will then scan and display  each receipt as they are given to me.
11. Wait for your letter from the Department with the result. I suggest you fax me a copy (+61 2 4659 6786) so we can put them online!
